Short-Term Recovery Care Forms
Short-Term Care Facility Certification of Care
This is a form for NHA06, NHA07, or NHA30 policyholders who are moving from one facility to another or who are going into a facility for the first time. Staff from the facility need to submit this form.

This is a form must be completed by facility staff. It is used to verify continuing care for NHA06, NHA07, or NHA30 policyholders. It must be submitted with each month’s bill.

American Medical Association, here’s how you can protect your practice from cyberattacks:
Protect mobile devices
Laptops and mobile phones are easy to steal. In fact, one laptop is stolen every 53 seconds. Make sure all your mobile devices are password protected and encrypted.
Keep software current
Running software updates regularly addresses security flaws in real time and serves as protection against malicious software. It’s equally important to update computers and operating systems that support practice work.
Install anti-virus software
Anti-virus software is specialized and can scan for, block, and remove computer viruses. Installing anti-virus software protection on all your computers is a critical step in safeguarding your practice from cyberattacks.
Secure your Wi-Fi
Your Wi-Fi password should never be shared. If you have a need for patients to access it, create a guest password. Also, make sure a firewall that can block malware is running on computers and servers.
Secure your passwords
Enforcing a strong password policy is necessary when it comes to keeping your practice safe. Passwords should always include a mix of letters numbers and symbols and be eight characters or longer. Each staff member should have a unique login and password that should never be shared.
